New students (excluding rising 9th VMS and MMS students)

Please bring the following with you:
1. Birth certificate
2. Social Security Card
3. Shot Record (on TN school form)
4. Copy of recent physical from doctor
5. Proof of Residence (electricity bill or lease agreement)
6. Custody Paperwork if parents are not married
7. Previous School Paperwork (Withdrawal Form/Transcript/IEP)
