Required Athlete Paperwork

Physicals must be completed on or after April 15th for summer conditioning/practices/games/matches/scrimmages and all 2019-2020 sports seasons. 

Please send your student to the clinic to sign up and get their physical packet. Physicals CAN NOTbe scheduled until the completed packet is returned. 

If your athlete is going to their private physician, they will also need to come by the clinic, to pick up the outside facility packet.
